Identity-Affirming Assessment

Be uniquely you.

An identity-affirming (e.g., a gender-affirming) assessment provides a supportive and respectful space to explore your experiences and needs. The focus is on understanding your unique story, celebrating your strengths, and reducing barriers to care. These assessments can help with access to gender-affirming medical care, school or workplace supports, or simply to guide next steps in your identity-based journey.

Our goal is to ensure you feel heard, validated, and supported every step of the way. We’re here to work for you, with you.

What to Expect

What is It?

An identity-affirming assessment (sometimes called a gender-affirming assessment) is a supportive evaluation designed to understand your unique experiences, identity, and needs. It creates a safe, respectful space to share your story, strengths, and goals.

Why Do It?

These assessments can help reduce barriers to care by providing documentation or recommendations for gender-affirming medical treatment, school or workplace accommodations, or legal processes. Most importantly, they affirm and validate who you are, while ensuring you have access to the supports that best fit your journey.

What Does it Entail?

You’ll meet with a clinician who listens with openness and respect. The process may involve conversations, questionnaires, and exploring your support systems and well-being. We move at your pace, centering your comfort, and provide clear recommendations or letters as needed. The goal is to leave you feeling understood, validated, and supported moving forward.

The Assessment Process

Interviews

We begin with a collaborative conversation to understand your experiences, goals, and supports. For youth, we meet with both the child and their caregivers.

Questionnaires

You may be asked to complete short questionnaires to better understand your emotional well-being, daily functioning, and support needs. Caregivers or other important people in your life may also be invited to provide input, if helpful.

Feedback & Report

Once the assessment is complete, we meet with you to share findings in a clear, supportive way. You will receive a written report or letter that includes practical recommendations and, if needed, documentation for medical, school, workplace, or legal purposes.
